Welcome back, Welsh families!

I look forward to working with you to keep all our students as healthy as possible and have an incredible 23-24 school year.

A few reminders for the start of the new school year.
  • All students new to our school district must have a Physical and Dental Exam. Please bring reports to school.
  • All students entering the 3rd grade need to have a Dental Exam. Please bring reports to school.
  • All students entering the 6th grade need to have a Physical. Please bring reports to school.
  • All students entering the 7th grade need to have a Dental Exam. Please bring reports to school.
  • All students entering the 7th must receive TDap and Meningitis vaccinations. Please bring updated vaccination records to school as soon as possible. This is a requirement of the State of Pennsylvania and MUST be done before October 5, 2023.
Listed below are some health forms that you might need throughout the year.
  • If your child goes to the doctor for a check-up, this Physical Exam form should be filled out by the doctor and returned to school. (MEH-1)
  • If your child goes to the dentist for a check-up or for dental work, this form should be filled out by the dentist and returned to school. (MEH-155)
  • If a doctor wants your child to take any medication while at school, the MED-1 form needs to be filled out by the doctor. This form needs to be signed by the parent or guardian and brought to school by an adult with the medication. (MED-1)
